Dear TRCS Parents/Guardians:

We are required by our St. Joseph County Health Department, and the Michigan Return to School Roadmap to have students and staff screened for COVID-19 symptoms daily, before entering school. For students we are accomplishing this requirement in one of two ways.

1. Parent/Guardian Consent to screen their child for COVID-19 symptoms before they enter school each day. This will be done at designated entrances.
2. Parent/Guardian agrees to screen their child for COVID-19 symptoms before they leave for school each day.

Please fill out, sign, and return one of the forms to your child’s school main office that indicates:

You are going to screen your child before leaving for school each day.

OR

You are providing us consent to screen your child at a designated entrance before entering the building each day.

** DUE Date for Returning Form: Wednesday, September 2, 2020.

Forms can either be submitted electronically below or can be printed and be returned to your student(s) school by email, fax, or in person.

**If you do not fill out, sign and return the form you will need to keep your student home from school until you do.

Please read carefully the Three Rivers Community Schools COVID-19 Student Screening Procedures. This document provides the symptoms you or school will be screening for, and the close contact questions that need to be answered.
